GoldenView Ultrasound understands the importance of proper prenatal medical care for both the expectant mother and the fetus. Therefore in order to provide our patients with an appropriate, meaningful ultrasound screening, GoldenView Ultrasound requires that you:

(i.) certify that you are under the care of a physician or other healthcare provider, and that you are not obtaining this ultrasound as a replacement for, or in lieu of, standard prenatal medical care;

We request that you present to GoldenView Ultrasound an acknowledgement of this notice, and provide us with documentation from your physician or health provider, prior to performance of the ultrasound. In the event you are unable to provide such proof prior to performance of the ultrasound, you assume sole responsibility for obtaining such information and relaying it to GoldenView Ultrasound as soon as practical following performance of the ultrasound.

As a further condition to receiving ultrasound services from GoldenView Ultrasound, you hereby acknowledge, understand, and agree to the following statements:

  • This ultrasound: (i.) is an elective procedure that I have voluntarily requested, and (ii) is not intended to take the place of a diagnostic ultrasound or any other test or treatment that has been or may be recommended by your healthcare provider.
  • Because of its elective nature this ultrasound is generally not covered by insurance. Therefore, advance payment is required.
  • The technician who performs this ultrasound, while qualified to provide such ultrasound services, is not a doctor, nurse, or healthcare provider, and cannot interpret, diagnose medical conditions from, or otherwise offer medical conclusions regarding the images produced.
  • As used by GoldenView Ultrasound, this ultrasound is intended to provide enhanced images for the purpose of viewing fetal movement in utero. The technician will make no attempt to guarantee a medically inclusive ultrasound or fetal wellbeing.
  • You understand that you are responsible for contacting your own healthcare provider if you have any questions concerning this ultrasound or any other aspect of your pregnancy.
  • You understand that the quality of the ultrasound and the video depends upon many factors including, body tissue content, developmental stage, and fetal position. You understand that GoldenView Ultrasound does not guarantee the quality of the Video, Photo, or other Image(s), or the ability to visualize any characteristics of the fetus.
  • As evidenced by your signature below, you understand that factors beyond our control may also affect the ability to accurately determine the gender of the fetus, and that GoldenView Ultrasound can provide no warranty or guaranty as to the accuracy of any such determination.
  • You further understand that while ultrasound in believed to have no harmful effect on the mother or the fetus, future research or other information may disclose harmful or adverse effects that are presently unknown.

If you are visiting us for Pregnancy Verification:

  • You believe that you are currently pregnant, in good health and that you are voluntarily choosing this ultrasound to confirm pregnancy.
  • As evidenced by your signature below, you understand that factors beyond GoldenView Ultrasound’s control may also affect the ability to accurately confirm the pregnancy, and GoldenView Ultrasound can provide no warranty or guaranty as to the accuracy of such determination.
  • You understand that you are responsible for contacting your own healthcare provider if you have any questions concerning this ultrasound or any other aspect of your pregnancy.
  • The Sonographer who performs this ultrasound, while qualified to provide such ultrasound services, is not a doctor and cannot diagnose medical conditions or otherwise offer medical conclusions regarding the images produced.
  • There is no refund for Pregnancy Verification, payment is due regardless of outcome.

GoldenView Ultrasound’s Policy on Images, Return Visits and Refunds – IL, MA, NY, TX

GoldenView Ultrasound makes no guarantees or promises that any images (still or video) created during your ultrasound session will be similar to those you might have seen elsewhere, in our clinic, or website or promotional material or that we can always meet your expectations. Every baby images differently, depending on its gestational age, position, fluid levels, and mother’s condition. We promise to make every effort to obtain the best possible images of whatever parts of the baby that can be seen.

Images

As a client of GoldenView Ultrasound I understand that there are many factors outside the control of GoldenView Ultrasound as it relates to images. This includes but is not limited to:

  • Baby is persistently looking face down, i.e., towards your spine, making it difficult to see the baby’s face
  • Baby’s placenta may be covering a body part including their face or there may not be enough amniotic fluid in the region being imaged (including the face) that may impact clarity of images.

Return Visits

  • If the ultrasound technician is unable to obtain any images ONE redo scan can be offered at the discretion of the ultrasound technician, at NO COST to the customer, and must be scheduled Monday – Friday within in 14 days of original appointment.
  • If I am unable/unwilling to return for a redo session, I understand I am still responsible for the cost-of-service GoldenView Ultrasound provided and no refund will be given, whether or not visit(s) produce favorable images.
  • We will email you all images and video that we were able to capture of any parts of the baby, regardless of quality.
  • For gender determination, if the technician is unable to determine the gender, we will bring you back on a weekday at no charge.
  • The ultrasound technician may require that you wait at least one week to reschedule the redo visit if they deem it necessary to get a more accurate view due to the size of your baby.
  • We will continue to bring you back at NO COST until we are able to get a good view of the gender.
  • I understand that GoldenView Ultrasound cannot guarantee the quality or quantity of the ultrasound images or recordings produced.
  • Redo visits on Any Session must be scheduled on a weekday Monday thru Friday. We do have daytime and evening hours for your convenience.
  • If for any reason, you can only come back on a Saturday or Sunday, there is an additional $25 weekend redo fee.

SonostreamLive-Internet Streaming

Sonostream is a 3rd party service offered to our clients in order to share your session online via internet streaming either Live or Recorded or both. If you have opted to use this service and have/are planning to invite others to view the ultrasound, please understand that they will be able to hear the audio in the room and view the ultrasound session (not the people in the room). Those that you invite(d) to view the session will need to pay a $12.95 fee to view the session live. If technical difficulties prevent them from viewing the session either live or recorded, then they must request a refund from Sonostreamlive.net customer support. SonostreamLive will only be viewed by those whom you invite and pay for the session and may be viewed by employees of SonostreamLive or GoldenView Ultrasound. Please note that this is unrelated to the Digital Video recording you may receive as part of your package. Lullaby music is recorded on the Digital Video purchased in the office or a part of your package, not the audio in the room. SonostreamLive records the audio in the room. I understand that by inviting others to view the SonostreamLive session that I release GoldenView Ultrasound of the responsibility of keeping the session private. I understand there may be situations outside of GoldenView Ultrasound’s control where the Sonostream may not be viewable by those I invited (bad internet connection, credit card declined, system down, etc.) I understand GoldenView Ultrasound will not redo or refund the images or Ultrasound session charges for any reason related to Sonostream. Sonostream Live will reimburse the internet streaming fee if they determine that it was not recorded to the server and is not available for viewing.

IN CONSIDERATION OF THE SERVICES RENDERED, YOU AGREE TO RELEASE GOLDENVIEW ULTRASOUND, ITS AGENTS, AFFILIATES, DIRECTORS, AND EMPLOYEES FROM ANY AND ALL CLAIMS OR CAUSES OF ACTIONS FOR INJURY, HARM, DAMAGE OR OTHER LIABILITY WHICH RESULTS FROM, OR ARE ALLEGED TO HAVE RESULTED FROM, THIS ULTRASOUND INCLUDING, BUT NOT LIMITED TO, THE FAILURE OF GOLDENVIEW ULTRASOUND TO ACCURATELY DETERMINE FETAL GENDER OR OTHER CHARACTERISTICS, OR TO ACCURATELY CONFIRM A PREGNANCY OR TO AND ANY DAMAGES OR INJURIES RESULTING FROM ULTRASOUND WHICH ARE NOT NOW KNOWN TO OCCUR.

Please note that there is a $50 no show fee for all unattended appointments unless cancelled or rescheduled 24 hours in advance of the appointment. I understand that the card used to hold the appointment will be charged under these circumstances and that I am responsible for this fee.

I verify the accuracy of the information above. I authorize GoldenView Ultrasound to disclose medical information to my healthcare provider if necessary. I agree that I am financially responsible for charges related to this ultrasound and once the ultrasound service has been received there are no refunds regardless out outcome.
